Abstract

Der klassische M. Paget der Mamille ist histologisch charakterisiert durch eine intraepidermale Tumorinfiltration, die von einem intraduktalen oder invasiven Mammakarzinom ausgeht, immunhistologisch durch eine regelhafte Überexpression von HER2 und klinisch durch ekzemartige Veränderungen von Mamille und Areola. Zu den Varianten mit davon abweichenden histologischen, immunhistologischen und klinischen Erscheinungsformen gehören der isolierte M. Paget der Mamille, der anaplastische M. Paget, der M. Paget mit Invasion und der pigmentierte M. Paget der Mamille. Differentialdiagnostisch ist der M. Paget der Mamille abzugrenzen gegenüber benignen Veränderungen, insbesondere der Tokerzellhyperplasie, dem Mamillenekzem und seltenen Dermatosen.

The classical Paget’s disease of the nipple is histologically characterized by tumor cell infiltration originating in intraductal or invasive breast carcinoma, immunohistologically by a frequent overexpression of HER2 and clinically by eczema-like changes of the nipple and areola. Variants with different histological, immunohistological and clinical features are observed in non-classical forms of Paget’s disease, such as the isolated Paget’s disease of the nipple, the anaplastic Paget’s disease, the Paget’s disease with invasion and the pigmented Paget’s disease of the nipple. In the differential diagnosis of Paget’s disease, benign changes have to be considered, including Toker cell hyperplasia, nipple eczema and rare dermatoses.
