Abstract

Eines der Hauptanwendungsgebiete therapeutischer Laser in der Medizin ist die Koagulation von Tumoren. Bei der Laser-induzierten interstitiellen Thermotherapie(LITT)wird die Laserenergie über speziell entwickelte Applikatoren im Kontaktverfahren in das tumoröse Gewebe eingestrahlt. In dieser Arbeit wurde ein Therapiesystem entwickelt, das die effektive und volumenkonforme Bestrahlung von Tumoren erlauben soll. Es umfasst ein Multifaser-System und neu entwickelte LITT-Lichtleiter, die im Gegensatz zu konventionellen Streuapplikatoren auf dem Prinzip der effzienteren Volumenstreuung beruhen. Mit verschiedenen Applikatortypen wurde in Gewebestudien die Steigerung der Effzienz bezüglich des induzierten Koagulationsvolumens untersucht. Dazu wurden Experimente unter Verwendung einer internen Applikatorkühlung durchgeführt und verschiedene Bestrahlungsstrategien getestet. Es konnte gezeigt werden, dass die gleichzeitige Verwendung von zwei Fasern gegenüber der aufeinanderfolgenden Bestrahlung eine Vergrößerung der Läsionszone von 60-70 Prozent zur Folge hat. Die in vitro-Experimente erfolgten unter Anwendung eines Kernspinresonanz-Thermometrieverfahrens, dass die quantitative Temperaturmessung in Echtzeit ermöglicht. Während die Einzelfaser-Anwendungen nur die Erzeugung von konvex geformten Isothermenflächen zulassen, können bei der Applikation von mehreren Fasern konkave Schädigungszonen induziert werden. Zusätzlich wurden mit einem mathematischen Modell die Temperaturverteilungen bei Multifaser-Applikation simuliert. Die Ergebnisse deuten an, dass die Multifaser-LITT eine effektive Methode zur konformen Behandlung von komplex geformten Tumorvolumen darstellt.

Translation of abstract (English)

Laser-induced interstitial thermotherapy (LITT) of tumors has become a major field of research for clinical laser applications. Light is most effectively applied in direct contact to the tissue by means of specially designed glass fibers leading to irreversible tissue damage by coagulation. In this study, a therapeutic system was developed including a multi-fiber system and a novel fiber applicator based on volume scattering. The experiments demonstrated, that volume scatterers distribute the light more homogeneously than conventional surface scatterers. The coagulation efficiency of different applicator types was evaluated as a function of temporal strategy. The simultaneous application of two fibers induced a coagulated lesion that was 60-70 percent larger than in a sequential application. The application of multiple fibers for the conformal irradiation of tumors by LITT has been investigated. Therefore a series of single-fiber and multi-fiber LITT experiments were performed in different samples of porcine muscle and liver tissue in vitro. For real time mapping of temperature changes in the irradiated tissue a new quantitative method of magnetic resonance imaging (MRI) thermometry has been applied. While single-fiber applications only induced convex shaped isotherms, concave structures could be generated using multiple fibers. A numerical model was used to calculate the temperature distributions induced by multi-fiber applications. The results indicate, that simultaneous multi-fiber LITT will provide the most effective strategy for conformal treatment of complex shaped tumor volumes while adjacent critical structures can be preserved.
