Abstract

In westlichen Industrienationen nimmt die medizinische und volkswirtschaftliche Bedeutung des Rückenschmerzes seit Jahrzehnten zu. Dennoch ist die epidemiologische Datenlage zu diesem Beschwerdekreis defizitär. Dieses Buch liefert erstmals für die Bundesrepublik Deutschland repräsentative epidemiologische Prävalenzdaten zur Verbreitung der „Volks-krankheit“ Rückenschmerz sowie zu seinen Risikofaktoren und zeigt Erklärungsansätze auf. Dabei scheint die Risikofaktorenstruktur scheint äußerst komplex zu sein. Berufsspezifische Belastungen sind gemäß der in diesem Buch präsentierten Befunde für das Schmerzrisiko ebenso bedeutsam wie ein ungünstiger, passiver Lebensstil, ein defizitäres Präventionsver-halten und das Vorhandensein relevanter Begleiterkrankungen. Es liefert somit wichtige Da-ten für die Versorgungsforschung (z.B. Schmerzprävalenzen, Risikofaktoren-Prävalenzen), Präventionsagenten (u.a. zu Nutzerstrukturen von Rückenschulen), Sozial- und Arbeitsmedi-ziner (durch die Identifikation von Hochrisikoberufe) und den klinisch tätigen Arzt (z.B. zu Begleiterkrankungen und Multimorbidität). Diese zitierfähige Online-Publikation stellt die Habilitationsschrift des Autors dar, welche an der Stiftung Orthopä-dische Universitätsklinik Heidelberg, Abteilung Orthopädie I (Direktor: Prof. Dr. V. Ewerbeck) entstanden ist und an der Medizinischen Fakultät der Ruprecht-Karls-Universität Heidelberg eingereicht wurde. Auf ihrer Basis wurde dem Autor im Mai 2007 die Venia legendi für das Fach „Sozialmedizinische Epidemiologie“ verleihen.

Translation of abstract (English)

Objectives: To investigate the prevalence of back pain in the German working population and the relationship between back pain and workplace stresses, lifestyle and social factors. Methods: The first National Health Survey of the Federal Republic of Germany was carried out between October 1997 and March 1999. It comprised a representative epidemiological cross-sectional study of the working population with total sample of 3,488 persons between the ages of 18 and 69. The participants took part in a medical examination and answered a self-rating questionnaire. The relationship between subjective back pain and workplace stresses, social, and lifestyle factors was investigated using bivariate tests and multiple logistical regression analyses. Results: The 7-day prevalence for back pain in the German working population was found to be 34%, and the 1-year prevalence was 60%. The odds ratios were significantly higher in women, persons of lower socioeconomic status, married and depressive persons, non-athletes and those who were overweight. Carrying heavy loads, maintaining a single working posture, environmental stresses, and psychological stress were the most significant work-related correlates of back pain. Conclusions: This study reports the first representative epidemiological prevalence data for back pain, and its correlates and potential risk factors, for the German working population. To reduce the negative impact of back pain, the most promising behavioral and conditional prevention measures in the workplace would be to reduce carrying stress and to vary working posture. In addition, a more active, athletic lifestyle plus the avoidance of overweight should provide an additional protective or preventive effect.
