Abstract

Zielsetzung: Bei der Strahlentherapie von Krebserkrankungen können akute Nebenwirkungen im Normalgewebe auftreten, die im schlimmsten Fall einen Abbruch der Therapie erfordern. Bisher gibt es keinen prädiktiven Test, der verlässlich voraussagt, wie strahlenempfindlich ein Patient ist. Die bis dato vorhandenen in vitro Testmethoden sind für den klinischen Einsatz nur wenig geeignet. Die DNA-Reparatur ist bei der Reaktion von Zellen und Geweben auf eine Strahlentherapie wichtig. Die mRNA-Expression in DNA-Reparatur-Genen (ATM, APEX1, BRCA1, BRCA2, ERCC1, MDM2, TP53 und TOP3A) von Krebspatienten vor einer Strahlentherapie sollte deshalb als möglicher Biomarker für eine Strahlenüberempfindlichkeit untersucht werden. In diesem Zusammenhang sollten zusätzlich Genvarianten in den Genen ERCC1, MDM2 und TP53 analysiert werden. Material und Methoden: In der vorliegenden Studie wurden 405 Prostatakrebspatienten untersucht, von denen 54 akute Nebenwirkungen zeigten. Im Rahmen der Arbeit wurde die Methode der relativen Real-Time PCR mit vorhergehender Reverser Transkription optimiert und für die Expressionsanalysen eingesetzt. Für die Untersuchung der Polymorphismen dienten die Analyse von Schmelzkurven mit Hybridisierungssonden und Restriktionsfragmentlängenpolymorphismus. Im Rahmen des Projekts wurden die Reaktionen der Patienten auf die Strahlentherapie in der Klinik erfasst. Über die Patienten wurde eine umfangreiche Datenbank mit epidemiologischen und anthropometrischen Daten sowie Angaben zum Lebensstil erstellt. Zusätzlich wurden in der Arbeitsgruppe anhand der Lymphozyten jedes Patienten zelluläre in vitro DNA-Reparatur-Parameter mit dem Comet-Assay bestimmt. Die Blutplasmawerte für folgende Antioxidantien wurden gemessen: Ascorbinsäure, die Tocopherole alpha- und gamma-Tocopherol und die Carotinoide alpha- und beta-Carotin, Canthaxanthin, Cryptoxanthin und Lutein. Ergebnisse: Die Patienten zeigten beträchtliche interindividuelle Unterschiede in der konstitutiven mRNA-Expression (37fach im Falle von MDM2). Die Expressionswerte einzelner Gene waren teilweise stark miteinander korreliert. Als Einflussfaktoren auf die konstitutive mRNA-Expression wurden die Antioxidantienkonzentrationen im Blut, vor allem die der Carotinoide, und die Prostatektomie identifiziert. Die Ausprägung eines bestimmten Haplotyps für das Gen ERCC1 zeigte einen statistisch signifikanten Zusammenhang mit der mRNA-Expression. Die zelluläre DNA-Reparatur-Kapazität und der Hintergrundschaden in den untersuchten Lymphozyten waren statistisch signifikant mit der mRNA-Expression bestimmter Gene korreliert. Die klinische Strahlenempfindlichkeit war nicht statistisch signifikant mit der konstitutiven mRNA-Expression assoziiert. Das höchste Odds Ratio von 2.08 (95% Confidence Interval: 0.9-4.78) für eine Strahlenüberempfindlichkeit zeigten Patienten mit einer erhöhten Expression von MDM2. Schlussfolgerungen: Die Regulation der konstitutiven mRNA-Expression ist ein komplexer Vorgang, der von endogenen und exogenen Faktoren beeinflusst wird. Um die mRNA-Expression als Biomarker einsetzen zu können, müssen diese Einflussfaktoren erkannt werden. Die vorliegende Studie identifizierte die Ausprägung eines Haplotyps des ERCC1-Gens und das Vorhandensein von Tumorgewebe als endogene Einflussfaktoren und den Status der Antioxidantien als exogenen Einflussfaktor. Darüber hinaus zeigte sich ein Zusammenhang zwischen der in vivo mRNA-Expression von DNA Reparaturgenen und der zellulären in vitro DNA-Reparatur. Die hier erhaltenen Erkenntnisse leisten damit einen Beitrag zum Verständnis der Regulation und Auswirkung der konstitutiven mRNA-Expression von DNA-Reparaturgenen in vivo und deren Nutzung als Biomarker für die Strahlenempfindlichkeit.

Translation of abstract (English)

Aims: Radiotherapy of cancer diseases might provoke acute side effects in normal tissue. The severity of side effects can necessitate the termination of radiotherapy. Until now, there is no reliable predictive test for radiosensitivity of patients and the available in vitro tests are not suited for clinical practise. DNA repair plays an important role in reactions of cells and tissues to radiotherapy. Thus, the mRNA expression of DNA repair genes (ATM, APEX1, BRCA1, BRCA2, ERCC1, MDM2, TP53 and TOP3A) in cancer patients prior to radiotherapy might be a promising biomarker for radiosensitivity. In this regard genetic variants in the genes ERCC1, MDM2 and TP53 were determined. This study was conducted to contribute to the development of a test for hyper-radiosensitivity. Materials and methods: Four hundred and five prostate cancer patients were included in our study with 54 showing severe acute side effects. An optimized version of the reverse transcription-coupled relative real-time PCR was used for mRNA expression analysis. Genetic variants were determined using melting curve analysis with hybridisation probes and restriction fragment length polymorphism analysis. The reaction of patients to radiotherapy was recorded in the clinic and a database containing epidemiologic, anthropometric and life style data of all patients was established. Within the project, the cellular in vitro DNA repair parameters of lymphocytes of every patient were analysed with the comet assay. In addition, the concentration of antioxidants in blood plasma was measured for: ascorbic acid, alpha- and beta-carotene, canthaxanthin, cryptoxanthin, lycopene, lutein, alpha- and gamma-tocopherol. The influence on mRNA expression was studied. Results: A considerable interindividual variation in constitutive mRNA expression was found between patients (up to 37-fold differences for MDM2). Significant correlations in mRNA expression levels between genes were detected. Influencing factors associated with constitutive mRNA expression were antioxidant concentration in blood plasma, mainly the carotinoids, and prostatectomy. A distinct ERCC1 haplotype showed a significant association with decreased mRNA expression. Significant correlations between DNA repair capacity, background damage and mRNA expression of specific genes were found. The constitutive mRNA expression was however not significantly associated with radiosensitivity. The highest odds ratio of 2.08 (95% confidence interval: 0.9-4.78) was found for patients with an elevated MDM2 mRNA expression. Conclusions: The regulation of constitutive mRNA expression is complex and influenced by endogenous and exogenous factors. For a successful use of mRNA expression as a biomarker, it is crucial to identify these influencing factors. In this study, an ERCC1 haplotype and the presence of tumor tissue were identified as endogenous factors and the concentration of antioxidants in the blood plasma as an exogenous factor associated with mRNA expression. In addition, the association of in vitro DNA repair capacity and in vivo mRNA expression was shown. These results might contribute to a better understanding of regulation and consequences of constitutive mRNA expression of DNA repair genes in vivo and their possible use as a biomarker for radiosensitivity.
