Abstract

Im Rahmen dieser Arbeit wurde untersucht inwieweit MR-Bildgebung an Patienten mit aktiver Tiefenhirnneurostimulation (DBS) durchgeführt werden kann. Die Arbeit befaßt sich im ersten Teil mit den potentiellen Risiken für die DBS-Patienten. Es wurden die Temperaturentwicklung an Elektrode und Zuleitung, mechanische Kräfte, die zwischen den Elektrodenkontakten abfallende induzierte Spannung, die Auswirkung defekter elektrischer Kontakte und die Funktion des Stimulators während der MRT untersucht. Dabei wurde ein erhebliches Gefährdungspotential für die Patienten nachgewiesen. Eine Sicherheitsempfehlung faßt zusammen, wie diese Risiken minimiert werden können. Es wird ferner gezeigt, daß die MR-Thermographie eine geeignete Technik für eine in vivo Temperaturkontrolle in Echtzeit ist. Der zweite Teil der Arbeit geht auf die Bildqualität der MR-Aufnahmen ein, insbesondere auf den Einfluß metallischer Implantate auf EPI-Sequenzen. Es werden drei verschiedene Korrekturansätze in Phantomstudien miteinander verglichen: Phasenkarten, Mehrkanalmodulation und Registrierung. Dabei erwies sich die Methode der angepaßten Phasenkarten als optimal zur Korrektur von EPI-Daten aus fMRT-Untersuchungen. Sie wurde in einer Fallstudie an einer Probandin getestet und zeigte sehr gute Ergebnisse.

Translation of abstract (English)

The aim of this study is to investigate to which extent MR imaging of patients with active deep brain stimulation (DBS) is possible. In the first part of the thesis the potential risks for the DBS-patients is examined. Measurements were performed to determine the temperature at the electrode and the connecting lead, mechanical forces, voltage induced between the contacts of the electrode, consequences of defect electrical contacts and the function of the stimulator. It could be demonstrated that - depending on the actual experimental setup - there is a potential risk for the patients. Safety recommendations to minimise the hazards are given. MR-Thermometry is shown to be an appropriate method for in vivo temperature monitoring in realtime. The second part of the thesis adresses the issue of MR image quality, in particular the influence of metallic implants on EPI-sequences. Three different correction methods are compared in phantom studies: phase maps, multi-channel-modulation and registration. The method of adapted phase maps proved to be the best correction method for EPI-data, which is used for fMRI examinations. The method was tested in an case study with a volunteer und showed very good correction results.
