Abstract

Die vorliegende Dissertation ist Teil des vom Bundesministerium für Bildung und Forschung geförderten Projekts „Informationstechnologie für eine patientenorientierte Gesundheitsversorgung in der Metropolregion Rhein-Neckar“. Ziel der Dissertation ist es, am Beispiel der Versorgung von Patienten mit kolorektalem Karzinom die Rahmenbedingungen für die Implementierung einer patientengesteuerten sektorenübergreifenden elektronischen Patientenakte in Deutschland zu evaluieren. Vor diesem Hintergrund werden im Rahmen einer qualitativen Untersuchung Voraussetzungen und Herausforderungen hinsichtlich der Anwendungsbedingungen professioneller Organisationen und des relevanten institutionellen Umfelds herausgearbeitet. Die Untersuchung setzt mithin den Fokus auf ausgewählte Leistungserbringer-Gruppen sowie weitere in diesem Zusammenhang wichtige Stakeholder im deutschen Ge-sundheitssystem (E-Health Experten). Ziel dieser Herangehensweise ist es, ausgehend von dem Erfahrungswissen der Leistungserbringer und der E-Health-Experten, typische verallgemeinerbare Strukturen, Prozesse und Praktiken zu identifizieren, welche die Implementierung einer PEPA beeinflussen. Die in diesem Zusammenhang geführten Experteninterviews wurden per Tonband aufgezeichnet, wörtlich transkribiert und inhaltsanalytisch ausgewertet. Im Zuge dieser Untersuchung konnten insgesamt 30 Experteninterviews durchgeführt werden. Dabei wurden zehn E-Health-Experten und 20 ärztliche wie nicht-ärztliche Leistungserbringer befragt. Die Voraussetzungen und Herausforderungen für die Implementierung einer PEPA werden mit dem Fokus auf vier Themen analysiert: 1) Patientenbeteiligung 2) Einrichtungsübergreifende Zusammenarbeit 3) Digitale Infrastruktur 4) Gesellschaftlicher Rahmen. Die Ergebnisse zeigen, dass sich die Befragten grundsätzlich von kommerziellen Lösungen distanzieren und sich für eine staatlich geleitete Implementierung der PEPA aussprechen. Der potenzielle Nutzen der PEPA liegt für die meisten in einer besseren Verfügbarkeit relevanter Gesundheitsinformationen, womit eine mögliche Verbesserung der Behandlungsqualität und das Schaffen von Zeitressourcen verbunden wird. Insbesondere für die Versorgung von Patienten mit einer hohen Krankheitslast sowie einer komplexen Versorgungsituation, beispielsweise aufgrund von seltenen Erkrankungen, chronischen Erkrankungen und Multimorbidität, wird die Nutzung einer PEPA als besonders gewinnbringend eingeschätzt. Sowohl traditionelle Rollenbilder im Arzt-Patienten-Verhältnis als auch mit der Profession des Arztberufes verbundene Handlungsprobleme kollidieren insbesondere mit dem Ziel der Patentenorientierung- und beteiligung und stellen für die Befragten relevante Hemmfaktoren im Umgang mit der PEPA dar. Auch die getrennten Leistungs- und Fachbereiche sowie die darin etablierten Strukturen, Prozesse und Verfahrensweisen generieren in Bezug auf interprofessionelle Zusammenarbeit und digitale medizinische Dokumentation objektive Handlungsprobleme bei der Anwendung einer PEPA. Für den Aufbau einer digitalen Infrastruktur als Voraussetzung für den nahtlosen Austausch medizinischer Informationen werden die typischen Bedingungen für die Gewährleistung technischer Interoperabilität sowie des Datenschutzes und der Datensicherheit als relevante Hemmfaktoren in Deutschland dargestellt. Gleichermaßen werden zentrale Mechanismen des deutschen Gesundheitssystems als mögliche Innovationsbremse für die Implementierung einer PEPA thematisiert. Die politisch vertretenen Inhalte und Zielsetzungen stehen dabei genauso im Fokus wie die für Deutschland charakteristische Selbstverwaltung und damit verbundene Prozesse und Verfahrensweisen. Die Ergebnisse zeigen, dass sowohl die E-Health-Experten als auch die Leistungserbringer der Implementierung einer PEPA grundsätzlich offen gegenüberstehen. Gleichermaßen wird deutlich, dass diese Grundhaltung nicht ohne weiteres in eine schnelle Einführung zu übersetzen ist. Die skizzierten Barrieren verzögern dabei die Implementierung der PEPA und werden nicht in jedem Fall einfach zu überwinden sein. Inwieweit die aktuelle Gesetzgebung in Deutschland Hemmnisse abbauen und die Implementierung vorantreiben kann, wird sich zeigen. Die Auseinandersetzung mit dem Thema macht mithin deutlich, wie hoch die Hürden und komplex die Voraussetzungen für die Anwendung einer PEPA in Deutschland sind. Ausgehend von der Fragestellung wird deutlich, wie wichtig der Einbezug des jeweiligen Arbeitsumfeldes, bestehender Organisationsformen und Strukturen sowie gesellschaftlicher Institutionen und Verfahrensweisen bei der Analyse der Implementierungsvoraussetzungen einer PEPA sind. Die Ergebnisse stellen explorative Befunde dar, welche in anderen Settings weitere Bestätigung finden müssen.

Translation of abstract (English)

This dissertation is part of the project "Information Technology for Patient-Oriented Health Care in the Rhine-Neckar Metropolitan Region" funded by the German Federal Ministry of Education and Research. The aim of the dissertation is to evaluate the conditions for the implementation of a patient-controlled electronic health record in Germany using the example of the care of patients with colorectal carcinoma. Against this background, a qualitative study is conducted to identify prerequisites and challenges of the implementation with regard to professional organizations and their institutional environment. The study therefore focuses on health care professionals and other relevant stakeholders in the German health care system (e-health experts). The purpose of this approach is to identify structures, processes and practices that influence the implementation of a PEPA, based on the experience of health care professionals and e-health experts. The expert interviews conducted in this context were tape-recorded, transcribed verbatim and evaluated by content analysis. In the course of this study, a total of 30 expert interviews were conducted. Ten e-health experts and 20 medical and non-medical health care professionals were interviewed. The prerequisites and challenges for implementing a PEPA are analyzed with a focus on four topics: 1) Patient participation 2) Cross-institutional collaboration 3) Digital infrastructure 4) Social context. The results show that respondents are generally opposed to commercial solutions and are in favor of government-led implementation of PEPA. For most, the potential benefits of PEPA include increased availability of relevant health information, with which is associated a potential improvement in quality of care and the freeing up of time resources. Especially for the care of patients with a high disease burden as well as a complex care situation, for example due to rare diseases, chronic diseases and multimorbidity, the use of a PEPA is considered to be particularly profitable. Both traditional role models in the doctor-patient relationship and problems associated with the profession of medicine collide in particular with the goal of patient participation and represent relevant inhibiting factors for the respondents in dealing with the PEPA. The strong separation of sectors and disciplines as well as the inherent structures, processes and procedures also generate objective problems for the implementation of a PEPA with regard to interprofessional collaboration and digital medical documentation. For the development of a digital infrastructure as a condition for the seamless exchange of medical information, the typical conditions for ensuring technical interoperability as well as data protection and data security are presented as relevant inhibiting factors in the German healthcare system. The politically represented contents and objectives are just as much in focus as the self-government characteristic of Germany and the associated processes and procedures. The results show that both e-health experts and health care professionals are generally open to the implementation of a PEPA. Likewise, it becomes clear that this basic attitude cannot be easily translated into a rapid implementation. The barriers outlined above will delay the implementation of PEPA and will not be easy to overcome in all cases. It remains to be seen to what extent the current legislation in Germany will be able to overcome obstacles and drive implementation forward. The discussion of the topic also illustrates how high the hurdles and complex the prerequisites are for the implementation of a PEPA in Germany. Based on the research question, it becomes clear how important it is to include the respective work environment, existing organizational forms and structures, as well as social institutions and procedures when analyzing the implementation requirements of a PEPA. The results represent exploratory findings that need further confirmation in other settings.
