Abstract

Das wissenschaftliche Interesse an Übergewicht und Adipositas ist aufgrund seiner weltweit zunehmenden Prävalenz sowie seiner Assoziation mit schweren Folgeerkrankungen stark gewachsen. Besonders im Bereich psychosozialer Einflussfaktoren besteht jedoch weiterer Forschungsbedarf. Aus diesem Grund wurde in einer repräsentativen Kohorten-Stichprobe (N = 5 102) mit 20-jährigem Nacherhebungs-Intervall der Einfluss von zu Baseline (1992-1994) erfassten Risiko- und Resilienzfaktoren auf den ebenfalls zu Baseline sowie zur zweiten Nacherhebung (2011-2012) ermittelten Body-Mass-Index (BMI) untersucht. Die Auswertungen erfolgten anhand einer querschnittlichen (N = 4 254) und einer längsschnittlichen Stichprobe (N = 1 306) auf korrelativer und regressionsanalytischer Ebene sowie durch Strukturgleichungsanalysen. Im Querschnitt wie im Längsschnitt erwiesen sich die Risikofaktoren Depressivität, Ärger und Zeitdruck als bedeutsam, der sozioökonomische Status ließ sich als protektiver Faktor identifizieren. Für letzteren sowie für soziale Unterstützung und Kohärenzsinn war in einigen Fällen ein moderierender Effekt festzustellen. Gesunde Ernährung und sportliche Betätigung trugen lediglich im Querschnitt zur Erklärung des BMI bei; zudem erwiesen sich die Lebensgewohnheiten als Mediatoren für den Zusammenhang diverser Risikofaktoren mit dem BMI. Die Befunde sprechen damit für den Einfluss psychosozialer Merkmale auf den BMI und geben Hinweise auf Ansatzpunkte zur Prävention, wie z. B. den Einsatz von Trainings zur Stressreduktion und einem „gesünderen“ Aggressionsabbau.

Translation of abstract (English)

Worldwide, overweight and obesity are increasingly prevalent public health problems . They are associated with a higher risk of developing various diseases. To date, little is known about the influence of psychosocial factors on obesity and overweight. Therefore, the study examined the relationship of psychosocial risk and protective factors on the participants’ body mass index (BMI) with a sample of N = 5 102 participants. Personality traits, measures of stress and BMI were assessed at baseline (between 1992-1994). The BMI, again, was measured after a follow-up-period of about 20 years. The data of the cross-sectional (N = 4 254) and a longitudinal sample (N = 1 306) were analyzed with correlations, regression analysis and structural equation modeling ). The results indicated that depression, anger and a facet of stress are risk factors for a higher BMI in both samples, while socioeconomic status protected against obesity and overweight. In the cross-sectional sample only, a healthy diet and physical activity were associated with a lower BMI, and these variables mediated the relation between some psychosocial risk factors and BMI. The results also indicated a moderating effect of social support, sense of coherence and socioeconomic status on the relationship between the psychosocial risk factors neuroticism, depression, anger and stress and BMI. These findings provide new evidence of an association between psychosocial factors and obesity, suggesting that trainings on stress reduction or a better anger management may be useful strategies to prevent weight-gain.
