Abstract

Die in vivo Diffusionsbildgebung ist ein komplexer Prozess, in dem der anscheinende Diffusionskoeffizient (ADC) unter Annahme freier Diffusion berechnet wird. Im Rahmen dieser Arbeit wurden Methoden entwickelt, die eine differenziertere Analyse der mikroskopischen Prozesse in der Diffusionsbildgebung erm¨oglichen. An einem 1,5T Ganzkörper-Tomographen wurde der im gesunden Pankreas gemessene Signalabfall durch die Parameter f und D der inkohärenten Bewegungstheorie (IVIM) quantifiziert. Mit einer pulssequenzbasierten Blutunterdrückung wurde der Perfusionsanteil f signifikant vermindert, wodurch der Einfluss der Perfusion auf das Diffusionsexperiment erstmals im Menschen bewiesen werden konnte. Es konnte außerdem gezeigt werden, dass f im Pankreaskarzinom signifikant reduziert ist (f = 8,6 ± 4,6% im Tumor vs. f = 25,0 ± 6,2% im gesunden Pankreas), wohingegen sich der Diffusionskoeffizient D kaum ¨andert (D = 1,15 ± 0,22 μm2 ms vs. D = 1,13 ± 0,46 μm2 ms ). Dadurch ermöglicht f eine bessere Differenzierung des Pankreaskarzinoms (P < 0,05) als der konventionelle ADC. Des Weiteren wurde ein Algorithmus für die Optimierung der Diffusionswichtungen (b-Werte) entwickelt, der die Stabilität des IVIM-Fits sowohl in Monte-Carlo-Simulationen als auch in in vivo Experimenten signifikant erhöht. Mit einem 9,4T MR-Tierscanner und einem Kryo-Spulensystem konnten Restriktionseffekte durch die Kurtosis-Bildgebung bei hohen b-Werten identifiziert werden. So war die anscheinende Kurtosis Kapp, die ein Maß für die Abweichung von der gaußschen Diffusion darstellt, im ischämischen Gewebe einer Maus 24 h nach Verschluss der Koronararterie (MCAo) im Vergleich zum gesunden kontralateralen Gewebe signifikant erhöht (Kapp = 1,25±0,10 vs. Kapp = 0,71±0,07). Die Ergebnisse der zum ersten Mal gemessenen Diffusionszeitabhängigkeit Kapp(Θ) und Dapp(Θ) weisen auf eine wesentliche Änderung der Zellrestriktionen bei Ischämie hin. Zudem bestätigten Untersuchungen von Schlaganfallpatienten an einem 3T Humanscanner die erhöhte Kurtosis im ischämischen Gewebe, wodurch die Möglichkeiten einer zusätzlichen Differenzierung innerhalb der Läsion deutlich wurden.

Translation of abstract (English)

Diffusion weighted imaging (DWI) is a complex process, in which the apparent diffusion coefficient (ADC) is calculated assuming free diffusion. In this work, novel methods were developed that facilitate a more specific analysis of the microscopic processes behind DWI. The signal decay was measured in healthy pancreatic tissue using a 1.5T MR-Scanner and the parameters f und D were quantified via the intra voxel incoherent motion (IVIM)-theory. The perfusion fraction f was reduced significantly using a pulse sequence with blood suppression. This verified the influence of the perfusion on the DWI-experiment for the first time in human. Additionally, it could be shown that f decreased significantly in pancreas carcinoma (f = 8.6 ± 4.6% in tumor vs. f = 25.0 ± 6.2% in healthy pancreas), whereas the diffusion coefficient D remained almost identical (D = 1.15 ± 0.22 μm2 ms vs. D = 1.13 ± 0.46 μm2 ms ). Thus, f improved the differentiation of the pancreas carcinoma significantly compared to the ADC. Moreover, an optimization algorithm was developed that improves the stability of the IVIM-fit considerably in Monte-Carlo simulations as well as in in vivo experiments. Using a 9.4T animal scanner and a cryo-coil, restriction effects could be identified at high b-values with kurtosis imaging. The apparent kurtosisKapp, which indicates the deviation from the Gaussian diffusion, increased significantly in murine brain ischemic tissue 24 h after a transient occlusion of the middle cerebral artery (MCAo) compared to the healthy contralateral tissue (Kapp = 1.25±0.10 vs. Kapp = 0.71±0.07). The time dependence of Kapp(Θ) und Dapp(Θ) was investigated for the first time and indicated a considerable increase of restricted water diffusion in ischemia. Furthermore, the increased Kurtosis in the ischemic tissue was confirmed by a patient study using a 3T human scanner and illustrated the possibilities of an additional differentiation within the lesion.
